At this time, the Texas Department of Health Services has included phase 1b for vaccination. You may submit your application if you fall into that category, however, please be patient. We have many front line healthcare workers that are in the queue to be scheduled. They will be given priority according to the Texas DHS.
Please read this form completely before sending your forms.
If you are a current patient at NTIDC and are in phase 1b, please complete the Immtrac form only and send to COVID.vaccine@ntidc.org and include in the subject line that you are a “current patient in phase 1b.”
We are still giving priority to phase 1a.
When you submit your forms, please include:
Your risk and job function in the body of the email
Two days and time frames that you have available.
As with all sites of administration, we will be following the guidelines set forth by them for priority in vaccinating. Please refer to the Texas Department of Health and Human Services website and the CDC for updates about phases and when certain groups can receive the vaccine.
Common symptoms for vaccine recipients after the first OR second dose:
Fever– and it can be high after either or both doses. Take Tylenol or Ibuprofen per manufacturers instruction unless contraindicated for your medical history or unless advised not to by your physician.
Fatigue– plan to rest for a few days after each dose.
Malaise
Body Aches– Tylenol or ibuprofen as stated above.
Headache– Tylenol or ibuprofen as stated above.
This vaccine is provided for you by the government; however, the administration of the vaccine (injection) will be billed to your insurance.
New Patient paperwork must be completed in entirety before we can schedule you for your vaccine.

The vaccine that we will be receiving first is the Pfizer mRNA vaccine. It is two doses. Day 0, then at least 21 days after the first.
We will give you your injection and monitor you for reaction for 15-30 minutes after.
